Information obligation
In connection with the fact that, as of 25 May 2018, Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), is in effect,
In accordance with Articles 13(1) and 13(2) of this Regulation, I inform you that:
- The controller of your personal data is BY RIESKE Monika Rieske, ul. gen. T. Kutrzeby 5/1U, 91-043 Łódź, NIP: 888-137-29-34, REGON: 100121574, email address: byrieske@yahoo.com(hereinafter: the Data Controller).
- The Data Controller processes the following categories of your personal data: first and last name; street; house number; apartment number; postal code; city; country; contact phone number; email; company name; and tax identification number (NIP). This data is collected from you at the time of placing an order and during its subsequent processing.
- Your personal data will be processed for the purpose of performing the contract concluded with the Data Controller, based on Article 6(1)(b) of the aforementioned Regulation, and for the purpose of providing you with other services by the Data Controller, based on the consent you have given.
- Recipients of your personal data may include entities providing services to the Data Controller, based on data processing agreements.
- Your personal data will not be transferred to a third country or an international organization.
- Your personal data will be stored for the duration of the services provided to you by the Data Controller under the concluded contract.
- You have the right to access your personal data, the right to request the rectification, deletion, or restriction of their processing, and the right to data portability.
- For reasons related to your particular situation, you have the right to object to the processing of your data. You also have the right to object without providing a reason when your data is used for direct marketing purposes.
- You have the right to withdraw your consent at any time, which will not affect the lawfulness of the processing carried out based on your consent before its withdrawal.
- You have the right to lodge a complaint with the supervisory authority if you believe that the processing of your personal data violates the law.
- Providing your personal data is voluntary; however, submitting it is a condition for the provision of services by the Data Controller.
- Your data will be processed in an automated manner. Your data will not be processed for profiling purposes.
